Age Well Services
District Nursing Team provides care in the patient’s own home (or Care Home) often for patients with complex needs and conditions. Care and treatments that can be completed in a patients’ home include vascular assessments, intravenous drug administration,(including by tube) and wound care.
If you need District Nursing Care your GP will refer you to the team.
Rapid Response Intermediate Care and Urgent Care multi-disciplinary team works with the Chalfont PCN to provide a 2-hour response for patients over 18 needing urgent care who might otherwise need hospital admission and a response within 2 days for people with less urgent needs due to their clinical condition, illness or social crisis.

Podiatry services provide a local clinic to diagnose and manage foot and leg problems such as painful corns and callus, in-growing toenails and foot ulcers, to help reduce pain, prevent deterioration of chronic conditions and increase your mobility.

Ear Wax Removal Clinics: two clinics provide services at the Centre, they re run by a private company and Age Concern GX Plus. The Age Concern Clinic is available for people over 65 on Mondays. The charge for one ear is £25.00 or £50.00 for both ears. If no wax is found, no charge will be made. The Private Clinic may charge higher rates.

Other Clinics and Diagnostics provided at the hospital before 2019 included some 30 different services. However, the development of Amersham Hospital as a Community Diagnostic Centre will reduce the need for some services (though improved transport arrangements may be needed) and demand for outpatient clinics may be reduced as more appointments are replaced by online access. There may still be some services best met by local clinics staffed from Wexham Park or BHT hospitals. We will update this web site as soon as we have news of any such developments.
The Health and Wellbeing Centre could provide a new model of integrated, whole person care, closer to peoples’ homes, providing outstanding services, supporting healthy communities and a great place to work. It could reduce the burden on GP, A&E and Outpatient services and help people return earlier, safely from hospital while achieving higher standards of health and wellbeing.
