Chalfonts Primary Care Network Services


The hospital site and adjacent clinics will also house support staff of the Chalfonts Primary Care Network. These are specialists supporting the work of the Allan, Hall and Misbourne practices close to the site (the Misbourne practice also provides a surgery in Chalfont St Giles).

Primary Care Networks (PCNs) were introduced in 2019 to reinforce GP and Nursing services, integrate primary care with secondary and community services and bridge a gap between GPs and Integrated Care Systems.  They are managed by GPs serving 30-50,000 patients, (in our case some 31,000 patients) and led by a GP (Dr Nicola Turner).The Chalfonts Primary Care Network employs specialist staff to support Primary Care services including: Paramedics Physiotherapists, Pharmacists, Social Prescriber Link Workers and a Frailty Support Team. Paramedics are trained to assess patients and provide basic treatment. Chalfont PCN GPs may ask a Paramedic to phone or text you to assess the treatment you require.

If you feel you need treatment but are unsure, you may wish to call NHS 111 (or contact online) or call your Primary Care Practice.


Physiotherapists a Chalfont PCN GP may ask a “first contact physiotherapist” to assess your need for treatment for problems with your muscles, bones, joints or ligaments. This can then lead to referral for a course of physiotherapy,(as provided at the Centre), advice on self-care or assessment of your fitness for work.


Pharmacists at the Chalfonts PCN monitor the prescriptions of patients and advise on adjustments to improve response to medication. Patients may get a text or phone call from a PCN Pharmacist to advise them of such adjustments.

If you feel you have an adverse reaction to your prescribed medicines, you can call your Primary Care Practice.


Social Prescriber Link Workers help patients address social concerns that affect their health and help people join community groups that can befriend them and support healthy lifestyles. Link Workers were engaged by the Chalfont PCN through a local community organisation, but it is recognized that an integrated team approach is needed, so the process has now been brought in-house and managed through General Practices.


The Frailty Support Team help people living in their own homes or care homes who lack mental and/or physical resilience, to recover from events like illness and injury, for example from chronic diseases or falls or the death of a loved one. They may require a range of healthcare and social support provided by an integrated team and a community support group.
